Small Group Lessons
Our programs are based on functional fitness exercises. We focus on strengthening all joints (ankle, knee, hip, back,, shoulder, and neck) and back pain. These exercises are good for people seeking to reduce or work on their bodily pain. We also help with weight loss for those looking to shed some pounds.
POLICIES AND AGREEMENTS
HEALTH DISCLOSURE & QUESTIONNAIRE
I understand that it is my responsibility to fully disclose to my instructor any health issues I have and medications I am taking that are relevant to participation in a strenuous exercise program, and to cease exercise and promptly report any unusual feelings (i.e., chest discomfort, nausea, difficulty breathing, apparent injury, etc.). Please read the following questions prior to your first session and inform your instructor of any questions you have answered in the affirmative: 1) Has your doctor ever said that you have a heart condition and recommended only medically supervised physical activity? 2) Do you frequently have pains in your chest when you preform physical activity? Have you had chest pains when you were not doing physical activity? 3) Do you ever lose your balance due to dizziness or do you ever lose consciousness?
4) Do you have bone, joint or any other health problems that cause you pain or limitations that must be addressed when developing an exercise program? (i.e diabetes, osteoporosis, high blood pressure, high cholesterol, arthritis, anorexia, bulimia, anemia, epilepsy, respiratory ailments, back problems, etc.). 5) Have you had any recent surgery? 6) Have you ever had any fear or nervousness in or around water.
24 Hour Cancellation Policy
By submitting this payment I agree to the following I understand that I am expected to attend every scheduled session and to follow instructions. Semi Private Lessons Lesson are to be shared. If one cannot make it to a lesson then there are two options; 1. reschedule the lesson 2. the one who can't make it forfeits their lesson Group Lessons We only allow one make-up lesson, the lesson will be schedule on the last Saturday of the month. I understand that Aquifit Swim School operates on a scheduled appointment basis for all sessions. Because my appointment time has been reserved especially for me, I am required to provide at least 24 hour notice when canceling a booked lesson. No charge will be levied should I cancel within the 24 hour notice given. Should I cancel a session without a 24 hour prior notice, I will be charged $20 rescheduling fee for the succeeding lesson. No call no show is a forfeited lesson. A maximum of 3 total cancellations will be accepted per swim package purchased. Any lesson cancelled 3 times over will be forfeited.
